Emplify Health and Homecare Hub Launch Innovative Transitional Placement Services to Address Aging Population Needs

Emplify Health, a not-for-profit regional health system, and Homecare Hub, a platform specializing in home care and housing coordination, have announced a partnership to launch Transitional Placement Services (TPS) in fall 2025. This innovative model is designed to bridge the gap in patient care during the transition from hospital to home or long-term care facilities, particularly for the aging population facing the so-called 'silver tsunami.'

The collaboration leverages Emplify Health's clinical expertise and Homecare Hub's technology platform to ensure patients receive personalized, high-quality care. The TPS model begins with a comprehensive evaluation by a multidisciplinary team, followed by the development of a tailored care plan. Homecare Hub's platform facilitates the coordination of care by connecting patients with suitable housing and care options, monitored in real-time to adjust care plans as needed.

The initial pilot program will launch in La Crosse, Wisconsin, featuring six dedicated beds in a Homecare Hub TPS home. This initiative aims to reduce the burden on hospital staff, enhance patient care, and improve hospital throughput and cost efficiency. The partnership is expected to save hundreds of thousands of dollars in avoidable patient stay time, resources that can be redirected towards patient care.

Andrea Hauser, RN, Vice President of Acute Care and Nursing Practice at Emplify Health, emphasized the significance of this partnership in addressing long-standing healthcare challenges. Similarly, Vipan Nikore, MD, CEO of Homecare Hub, highlighted the complexity of solving the post-acute care crisis and the importance of innovative solutions like TPS.
