Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant will support a collaborative project that leverages Avident Health's proprietary MyNavigator digital platform to improve care delivery for patients diagnosed with metastatic breast cancer (mBC). The initiative aims to bridge communication gaps between clinic visits by providing personalized, patient-friendly education aligned with NCCN Guidelines for Patients, increasing enrollment and utilization of Unite for HER's free supportive services, and equipping informal family caregivers to foster shared decision-making, treatment adherence, and enhanced quality of life.
Metastatic breast cancer requires complex, ongoing treatment management where adherence, symptom monitoring, and whole-person supportive care are critical. Yet patients and their loved ones often struggle to navigate clinical guidelines and access community-based supportive resources outside traditional clinical appointments. The MyNavigator platform addresses these challenges by serving as an interactive, home-based companion that delivers tailored NCCN Guideline education, translating complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey. It also streamlines access to integrative supportive care by directly integrating Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts.
Uniquely, the platform engages informal caregivers, such as family members and daily care partners, in a secure, proactive communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home. Additionally, it enables proactive care team interventions by capturing patient-reported check-ins and symptom trends to alert clinical teams early, preventing avoidable complications and reducing acute care utilization.
